Neo Geo PCB - what buttons?

I have a original Killer Instinct arcade machine which has 6 buttons + start for each player.

If I where to swap out the PCB for a Neo Geo PCB, what buttons would the Neo Geo use?

A single slot MVS should plug in and you'll get buttons ABC on the KI punches (button D will not be connected). Start, coin and service buttons will work normally as well.

if this is a multi slot MVS you'll need an adapter because they uses a non-standard JAMMA pinout.

So the D button doesn't get assigned to any of the kick buttons? It's just not there unless I wire it?

Yes you would need to add a pin to the JAMMA harness and run the wire yourself.
