I'm wiring a custom arcade cabinet and I'm confused about the output from my audio switcher and input to a car amplifier.

Here's the diagram:

The source devices have line output 3.5mm stereo jacks and the Extron Audio Switcher has 3.5mm stereo input jacks.

However, the Extron Audio Switcher output jacks are:
Left (+, -)
GND
Right (+, -)

The manual discusses balanced versus unbalanced audio. Which should I be using?

After the audio switch I'm installing a 10k/100k dual-gang audio potentiometer for volume control. I think I know how to wire this but I'm not certain.

Finally, my audio amplifier (JBL DA4002, car amplifier) has RCA input jacks labeled "Pre-amp Line Level Input" but it also has 4 terminals labeled "Floating Ground Speaker Input."

Which should I be using?

Sorry for the questions and I greatly appreciate any help at all!

If you want good sound output, I would use the balanced output, it will provide a cleaner audio signal compared to unbalanced. That’s why there’s an extra ground prong on your device as well.
