VCC1 should be positive voltage. (I think what usually happens if VCC is disconnected is that power goes from the pins through the ESD protection diodes, which results in glitchy operation.)

I'd try to find which pin exactly needs to be disconnected for it to work, then check for short-circuits on that pin.

You could probably make an DB15-DB15 adapter that adds out 1-3. Or solder them directly to the controller.

It is in place of the normal IO test screen. I am not sure if all BIOS versions support this, though.

Just to be clear, with pre-populated I mean all the off-the-shelf stuff like capacitors and resistors that the PCB manufacturers have in their pick-and-place machines anyway.

IMHO getting a pre-populated board is a good idea, not only to reduce the work but also to cut down on avoidable errors. And unless you get a stencil and a reflow oven it'll probably be neater as well.
