The material is pretty easy to find -- just google "0.5mm PVC sheet" and you will get lots of suppliers.
As @ImportantBet bet mentioned, we were talking about reproducing this part for the Egret/AWSD cabinets. It's pretty straightforward, but PVC can't be (safely) laser cut. Looking at your NAC sheet, I'm guessing this won't be a problem as it's just straight edge cuts and a few holes. This could easily be done by hand.
I plan to experiment with a few different materials (PC and PS first) so I can laser cut a stack of these for the Egret marquee holder. It's about as simple as the one in your picture, but it has a couple slots I'd prefer to do via CNC.
