Thanks for the link. Have looked at the schematic and the reader PCB is a Sega 839-1233 on this panel and it has a different pinout to the one on ghost squad.

Based on the ICs you have pulled and presumably verified, A dry joint on the board somewhere or touching pins on the solder side. This may sound trivial but ensure all the pins on the 20 pin header are all straight vertical (spent ages troubleshooting one board to discover that was the issue)...
